Is it on a W-2, 1099 (what type of 1099), etc.? And what state? Is it Utah by any chance? i.e., is it called a Utah Payer withholding ID? Is it an error on Utah "Form TC-40W?"
It is Utah and it was on my W-2. It just says Employers State ID Number. Yes, it is an error on Utah form TC-40W

When entering the W-2, the Utah Employer/Payer state withholding ID number in Box 15 must be 14 characters in length. It will be 11 numbers then ends with the additional letters wth. Do not use any hyphens/dashes. Several Utah users say they have to use lower case wth to get it to work.
If there are any 1099's with an employer/payer withholding ID number, you may have to use lower case on those, too.
