I dont have anything in boxes 12, 13, 14 and it's still asking for the state payer #. any suggestions this year?

Thanks for your response. On my 1099-R, which is from my retirement system in California, there was no withholding withheld. To file my state tax form, Utah is requesting an 11 digit code that ends with WHT. I tried the CA state number, but that doesn't work. There is nothing on my form that has an 11 digit code or ends with any letters.
I was really frustrated yesterday because I couldn't file the federal either and the computer I was using couldn't recognize my printer so that I could print my tax filings.
If no state tax was withheld, you can leave boxes 14, 15 and 16 blank.
The program wants all three entries if you fill in any of them. If you leave these three blank, it will not change your return and you will be able to e-file.

Just to be clear (and to follow the comments above), do you have entries in boxes 14 or 16? If neither, then delete any entry in box 15.
If that doesn't help, look in the upper left hand corner for the Payer, and contact them to ask what their box 15 value is. It's not like they don't know.

For form 1099-R, if you don't have any state tax withheld in box 14, you can leave boxes 14,15 and 16 empty.
