You should contact your HR department to ask if the Maternity Leave benefit is included in your W-2 (we would have no way of knowing this).
Note that e-filing at the IRS does not open until January 27th, so there is no rush to file just yet. Better to wait a week or two so that you can be sure of the answer and not have to file an amended return if you get another form.
