I care for children in their home as a licensed exempt childcare provider

if no taxes have been taken from my paycheck should my w2 have zeros in all the withholding spots?  Or do I deduct what should have been taken and have the parent send to the IRS?

Yes, if no taxes have been withheld from your paycheck, your W-2 should show zeros in all the withholding spots. You do not need to calculate or deduct taxes that should have been withheld or have the parent send them to the IRS. Instead, you should report the wages shown on your W-2 and pay any income taxes due when you file your tax return.
