We only had a healthcare marketplace plan for 5 months before switching to a Christian Ministry plan. Do I only fill out the 5 months that we had? What about rest of time

Leave all the columns for the months where you did Not have Marketplace Insurance blank (empty)

Assuming that "Christian Ministry Plan" is a non-insurance "Healthcare Sharing Ministry", you will initially say you only had health insurance for PART of the year.

Then it will ask what kind of insurance you had, and when you say you had insurnace from the Healthcare Marketplace, you will enter your 1095-A.

 Then it will ask you if you qualify for a waiver to the penalty for your uninsured months.  That is where you will indicate that you were part of a Healthcare Sharing Ministry.
