It shows my turbo tax fees got paid. But my refund hadn't even been approved yet....I opted to pay it out of my refund. I'm confused

When you signed up for Pay with Refund, you agreed to let TT take its fees if your refund was delayed beyond a certain point.  TT also sends you several emails before it does this, so please check your spam email.

If you choose to pay your fees from your federal refund, then TurboTax will show that the fees are "paid" ----but they are not really paid until the IRS issues your refund and the third party bank extracts the fee and sends you the rest.   

In the event that there is a too-long delay in processing by the IRS, you agreed to allow the fees to be taken out of your account even before the refund is issued.

You signed an agreement that says TT can withdraw its fees from your account.  TurboTax would have sent several emails to you before they took the money out of your account.

Then what is your question about?    TT assumes your fee will be paid since you have signed an agreement that says they get their payment from your federal refund OR from your account if your refund is delayed.    TT considers it "paid" --- you will receive your income tax refund when the IRS issues it.
