If there is a state abbreviation "UT" at the front of it on your W-2, do NOT include the UT and space as part of the number.
The UT withholding ID number should be 14 characters long--11 numerical digits followed by the letters wth. For the wth use lower case (I know it's a weird quirk). Example 12345678901wth
If it's withholding from mineral payments, it may end in "wmp."
If you have multiple W-2's, 1099 forms, etc., that have UT withholding, you must use the same format and lower case on all of them.
