If I mail RI State return, do I need to attach 1099-B/DIV/INT with my W-2?

What are other form I need to attach? Do I need to attach my health coverage?

If I mail RI State return, do I need to attach 1099-B/DIV/INT with my W-2?

No, you do not. All you need to make sure to attach to your return is the Schedule W that was printed with your return. It lists all of your 1099, W2, etc. information. Keep those forms for yourself. 

I have also included a link to the instructions at the bottom of this page.

If you are claiming a refund, mail your return to: 

Rhode Island Division of Taxation

If you are making a payment, mail your return to:

No, you do not. All you need to make sure to attach to your return is the Schedule W that was printed with your return. It lists all of your 1099, W2, etc. information. Keep those forms for yourself.
