If i have health choice for insurance, what do i put on my form

im unsure on what to put for my ins on my form. do i put shop? i have health choice from my work

If you have health insurance from your work, then you should select that you had health insurance all year.  Do not choose Obamacare and it is assumed you do NOT have Form 1095-A.
