I was told that I do not receive 1099-div forms, after checking with my financial institution

if they say no 1099-divs are issued, did you have just a small amount (less than $10 they're not required to issue) . if less than $1 just skip this section. if more create a 1099--div in the name of the broker and enter the Y-T-D amount
