If I am filling out my 1099misc and had a contract with the company for 3 months before I was permanently hired what do I put for the business name and address?

If you are filing Schedule C for the time you were an independent contractor before being hired permanently, you would use your own name and address for the business name and address on the Schedule C. You, yourself, performing the work for the company that paid you, are the business.
