Maybe the state tax ID of your employer was changed during the tax year?

Check the state's Employer Identification Number (EIN) in box 15 for both entries.  If the state's EINs are different, you will have to make two entries (one for each State EIN) in boxes 16 and 17 in TurboTax. If the EINs are the same, add the amounts in box 16 together and box 17 together for both lines on your W-2. Enter the total amounts in boxes 16 and 17 in TurboTax.
