Possibly, you must report as income any amount you receive for your disability through an accident or health insurance plan paid for by your employer.   If you pay the entire cost of a health or accident insurance plan, don't include any amounts you receive for your disability as income on your tax return.  If you are not sure if you or your employer paid for your disability insurance contact your employer.

It depends upon who paid you the disability income: Social Security, an employer plan, a state plan, or a private insurance plan. Please provide more details. Did you receive a W-2 or 1099 for it?
