Yes, you do need to include that W-2 since you paid taxes during the time you were employed there.
Will you receive a W-2 from that employer ?
By law, you are required to report this wages and withholding along with your other W-2 / income on your tax return.
If you will not receive a W-2 from them, report it as a miscellaneous income in the program:
1. Federal taxes / Wages & Income -Less Common Income ( very last section on the screen) /
2. Miscellaneous Income , 1099-A, 1099-C- Start /
3. Miscellaneous Income- Other income not already reported on a Form W-2 or Form 1099- Start /
IRS does not forgive a cent
