I need proof that I filed a tax extension (form 7004) where can I download that form?

7004 is not for individuals. it's for corporations partnerships, trusts and estates and certain other forms.

4868 would be the form for self-employed.

however, if you had one of the above entities for which you filed a 7004 it should be with that tax file.
