For individuals, the fee may be reported on the Louisiana individual income tax return, or on
Form R-19000, Electric and Hybrid Vehicle Road Usage Fee.
See the Instructions for Form R-19000 for more information.
Mail to:
Louisiana Department of Revenue
P.O. Box 201
Baton Rouge, LA

Please mail in the Form-19000 to Louisiana Department of Revenue, P.O. Box 201, Baton Rouge, LA 70821-0201. Include a copy of your IT-540, showing the fee was included on Line 22B of your tax return.
