I just noticed both my state and my federal tax refunds were Offset for the same amount’s I owed to the ira

Unfortunately, it happens from time to time where both the IRS and the State offset your refunds for the exact same debt.  You will need to contact the Bureau of the Fiscal Service (FBS) so that you can be refunded you for the double payment: 800-304-3107.
