I don't know amout of $$ sent with form 4868 in 2018

Can't find any record in bank history.

Jpeg of 4868 form: Line 4: $750

                                    Line 5: $800

                                    Line 6: $0

                                    Line 7: Blank

I believe I sent something because I believed I had to. Foggy memory says maybe $50.

I'm stuck & way behind with little time left. Please help me.

Since Line 7 is blank then you did not enter any amount to send with the extension request Form 4868

Just complete and e-file the 2018 tax return before October 15, 2019.  After that date the 2018 tax return can only be printed and mailed.

There is no penalty for filing a return after the due date if you are owed a tax refund or there are no taxes owed.

Did you use a credit card instead of your bank account?  Have you checked Credit card statements for dates before April15, 2019?

If you think you only sent $50, then that was not much--the extension does not give you extra time to pay your tax due.  It only gave you extra time to get your documents ready and file them.  But you have not said how much the Form 1040 says you owe, so we do not know if $50 could have been reasonable.

Has anything been entered already into your tax return?

When you requested your extension you were supposed to pay your estimated tax due.  Did you enter the payment you made into your tax return?  Go to Federal>Deductions and Credits>Estimates and Other Taxes Paid>Other Income Taxes  This will show up on line 17 of your Form 1040

you can go to irs website and either click on "view your account" or "get your tax record"

if you don't enter an amount, but you did pay and the IRS got it,   the IRS will likely adjust your return giving you credit for the extension payment.  if you paid by check for this to happen  your social security number would need to be on the check
