I am supposed to report a form 8959. From whom do I receive this form, my employer or the IRS? When does this form become available?

I filled in all my info and Turbotax tells me that this 8959 form is not available yet and will inform me when it becomes available. This is the first time I hear about this form as my income is now to the level is requires it. I do not understand who gives me the form.

That form is generated automatically in TurboTax based on your entries in the program. It should be available soon. If you are using the desktop version of TurboTax you need to do the updates to make sure you have the current version of the program. Just go to Online and then Check for Updates on your menu bar.

Thank you for the response. I guess I will just wait for Turbotax to receive then.

Thank you.

I have latest TurboTax updates installed. Form 8959 Line 24 is not carrying over to Form 1040 Line 25c.

IRS Form 8959 Additonal Medicare Tax is available per forms availability.

Starting with the 2013 tax year, you may be subject to an additional 0.9 percent Medicare tax on wages that exceed a certain threshold. The form is generated automatically in TurboTax based on your entries in the program.

Is this form working correctly for you now?  @LW22

The 2020 form is still not working - the amount from Form 8959 Line 24 does not carry to Form 1040 Line 25c. Latest updates were installed today.

It appears to be working in Home and Business.  I would suggest reviewing your W2 input again to make the Medicare is 1.45% of medicare wages.  The other option is to delete the W2s and re-enter them in TurboTax.
