Yes, you would still fill out the information, however, you would answers the question as if you are the "business". Whenever you receive a 1099-MISC with income in Box 7, the IRS considers you self-employed/an independent contractor/business. Therefore, you would answer the questions with your personal information if you don't have an established business. You would enter you home address/mailing address as your business address, the business name would be your name, you would enter a brief description of the the type of work you performed, etc.... Most likely since you were an independent contractor most of the questions will not apply to your situation (no employees, no EIN, no business code, not required to issue 1099's and most contractors use the "cash" method)
