You selected to pay the TurboTax account fee of $19.99 with the federal tax refund which is an option.  When the pay with federal tax refund was selected you were shown the cost for that option, $39.99.  And several more pages where you had to agree for having the payment deducted from the federal tax refund and again showing the charge for that option.  See Screenshot.
