I worked in 2023 and had HSA with HDHP Made contributions all 12 mouths total of 1925 . I retired 1/1/2024 Had SSI and medicare start 1/1/2024 filed 2023 taxes got credit for all contributions. Problem is Medicare part A  went back to my birthday 11/25/2023 so Contributions off 200 I did And the 150 my Company did were done when I had medicare. So how exactly what forms and everything do  I  use to amend 2023 return . I did a amended return on my H and R block software nothing changed

HSA

The maximum HSA contribution in 2023 was $4,850 for someone over 54.  Prorated for the months you were eligible to contribute (11 months), your maximum 2023 HSA contribution was $4,850 divided by 12 months in the year times 11 months of eligibility = $4,446.  It sounds like your contributions were well under the limit for the time your were eligible for an HSA, and thus you do not have to amend your return.
