How to fill the 1095-a with 3 different self income sources?

Hi

When I filled out the form "Deduction & Credits,--- Medical----Affordable Care Act( Form 1095-A)" , I chose" I am self-employed and bought a Marketplace plan". I think I have 3 self-coming sources, but I ONLY CAN choose one to continue. So what can I do？ Thanks
