How to account for Election Judge (no w2) and additional federal withholding?

I have w4 for election judge in minnesota (total $491.25) and had $30 taken out in additional federal withholding.

I never got a w2 because it is under $600.

From what I am reading on this forum, I should report the $491.25 in other income.

1. Is that correct?

2. How do I account for the additional $30 I had taken out as additional federal witholding?
