HOW DO YOU ENTER LOSAP WARD IN NYSTATE TAX

If you're referring to a Length of Service award, how was your award reported? 

That is, is it shown on an IRS form such as a Form W-2, 1099-MISC or 1099-NEC?  

Was the award based on employment with a government entity, or a private company?
