How do I correct my Federal Tax Extension form?

I received a message that My Federal Tax Extension form was rejected because of an error. They didn't say what the error was.

You have until midnight on April 21st to resubmit your extension. Make sure your name, date of birth and SSN are correct. Most of the times the rejection is due to a mismatch between your name, SSN and date of birth.
