Health Insurance tax carryover

I have a tax carry over originating from Schedule E P2 for SE Health Insurance premium.  Where do I report it in my taxes?  I see that my accountant had it on a Basis WR and passthrough form.  I don't know why and he passed away unfortunately.  Can anyone help?

Can you clarify what a Basis WR is?

Sorry, that was a typo.  The originating form was a Basis Worksheet (WS).  But there is a taxcarryover to 2022 of the SE health insurance premium.  Where do I post that?

The disallowing form is Basis Worksheet and the origination form is Schedule E P2.
