I received my 1099-R and entered all information corresponding to the boxes. I am left with an amount with no numbered box labeled "Health Insurances Premiums" n between box 12 & 13. There doesn't seem to be a place to add that information in. I have information in box 5 that is labeled "employee contributions/ design.roth contributes/ insurance premiums" that has a different amount.
Any assistance is greatly appreciated.

1. Is it possible that this number between 12 & 13 actually belonged in box 11?
2. Is it possible that box 5 has a number for one purpose like "Employee contributions/ Designated Roth contributions" so the health insurance premiums had to get pushed elsewhere?
I have not seen this situation before; you might be best off calling the issuer of the 1099-R and asking just what they meant by this.
I called the Issuer and they let me know this is normal and how they print their 1099-R's. Thank you!
But what did they say what this amount meant?
Unfortunately, they weren't very helpful and said this is how the form is printed every year. Then told me to call a tax specialist.
I'm sorry but we tax specialists won't be able to help if there are made-up numbers on the 1099-R that the plan administrator doesn't know what they are.
OK, health insurance premiums would normally go in box 5, as you would suspect. However, since there is already a number in box 5, I would not want to overwrite a number on the form if I didn't know what it was.
Since I don't have a crystal ball, could I ask you to call the plan administrator again and ask what is the value in box 5:
1. employee's contributions, <- these are after-tax contributions to your pension plan.
2. designated Roth account contributions, or
3. health insurance premiums
Maybe the people you are talking to know what they put in box 5, and we can back into what to do with the unnumbered "box".
