Go ahead and enter the information on the 1099-SA as printed, even if it's out of date.

It's OK, because the IRS already has a copy of your 1099-SA, and it's good that your copy and their copy match.

Besides, the only place where your up-to-date address really matters is in the Personal Info, which is printed on page 1 of the 1040. This is the address that the IRS will use to mail you any correspondence, so it has to be correct.
