Form 8915 missing from Last years tax return/not included in copy of last years tax return

I reported a 1099-R IRA Distribution and choose to spread the proceeds for 3 years 2020, 2021 and 2022. In 2020 the 8915 E tax form was included in my return. However in 2021 there is no 8915 tax form.(even though the numbers were reported) So now I do not have the numbers needed to complete this years tax return, since I don't have a 8915 in last years return.

I was told by Turbo Tax representative that I spoke to on the phone that I'm unable to see it in my downloaded saved to my desk top tax return because it is not currently available. That seems strange, but I honestly can't remember if it was part of my return when I completed it last year. I assume it was or I would have noticed, even though I was frustrated as to how long it took to be available. I even tried to amend last years return just to see if i had done it incorrectly, and I had not. So why isn't the 8915 included in my last years return?

Form 8915 missing from Last years tax return/not included in copy of last years tax return

Do you remember entering information in the retirement section on your 2021 tax return:

  1. Login to your TurboTax Account
  2. Click on the Search box on the top and type “1099-R”
  3. Click on “Jump to 1099-R”
  4. If you do not have any 1099-R answer "No" to "Did you get a 1099-R in 2022?" (If you have any 1099-R then enter all 1099-R and after entering your last 1099-R click "Continue" on the “Your 1099-R Entries” screen)
  5. Answer "Yes" to the "Did you take a disaster distribution in prior years or repay…" screen
  6. Answer "Yes" on the "Did you take a 2021 Qualified Disaster Distribution" screen
  7.  Check the box next to "If this was a Coronavirus-related distribution reported in 2020 check here" and enter your information.

Please sure you enter the amount from Form 8915-E, line 9 twice (if it was not from an IRA) or enter the amount from Form 8915-E, line 17 twice (if it was from an IRA). This amount should be 1/3 of your 2020 COVID distribution.

It took a while for the 2021 Form 8915-F to be ready last year.

If you do not have it included on your 2021 return then you will have to amend your 2021 tax return.

After you have fixed the 2021 tax return you will be able to enter the 3rd portion on your 2022 tax return:

  1. Login to your TurboTax Account
  2. Click on the "Search" on the top and type “1099-R”
  3. Click on “Jump to 1099-R”
  4. If you do not have any 2022 1099-R answer "No" to "Did you get a 1099-R in 2022?" (If you have any other 1099-R then enter all 1099-R and after entering your last 1099-R click "Continue" on the “Review your 1099-R info” screen)
  5. Answer "Yes" to the "Have you ever taken a disaster distribution before 2022?" screen
  6. Answer "Yes" to "Did you take a 2020 Qualified Disaster Distribution?"
  7. Check the box next to "If this was a Coronavirus-related distribution reported in 2020 check here" and enter your information.

Thank you for responding. I did enter it(maybe not twice). I started to Amend the 2021 and there were no changes. I’ll do the Amendment again(entering the numbers twice). Btw I filed my taxes March 14 last year, I was aware of the delay in the form..
