My husband had two HSA accounts for the first time last year. Do you file a separate Form 8889 for each account? One account had contributions but no distributions, the other account had both. We didn't get a 1099 SA from the account that had no distributions. Will not having that form cause problems with TT? Also he did the one time rollover from a traditional IRA to the HSA. Does that amount go on line 10 of the 8889?

There is only one Form 8889 to be filed for your husband's HSA's. Your husband's two HSA accounts are treated as a single account for tax purposes.
Only distributions are reported on Forms 1099-SA. If there was no distribution from a particular HSA, there will be no Form 1099-SA from that account.
Yes, TurboTax will include the HSA Funding Distribution on From 8889 line 10.
