The Form 1095B is a federal tax form that is for informational purposes only. This form does NOT need to be entered into Turbo Tax. Form 1095B provides information regarding the health coverage that you had in 2016 and can be helpful in answering questions on your tax return regarding your coverage.
However, Massachusetts has their own health care requirement and the MA 1099-HC provides information on what type of health coverage was provided during 2016 and whether it is considered minimum creditable coverage for Massachusetts. If you did not receive a MA 1099-HC form, you can request one if you need it to address the questions in the tax return. Please refer to page 1 of the 2016 Massachusetts Schedule HC Health Care Special Section on Minimum Creditable Coverage, for additional clarification. http://www.mass.gov/dor/docs/dor/health-care/2016/hc-instructions.pdf
