Fed and sState rerjected due to multiple HSA accounts

I have submitted multiple times and verified that all the information is correct including deleting From 8889T or what ever it is, and then answering the interview for HSA all over again. My wife and I are under the same HSA account I am the only one making contributions through my company. It appears i over contributed but i have no way to withdraw the contribution in the old HSA since there is not money in it. How do i get my Returns accepted??  TurboTax is not helpful at all.

I copied this from a another thread where I asked a similar but totally related questions and got my Fed. accepted today in just a few minutes. Still waiting for the State but i am sure I am in the right process now:

(Again, the following worked perfectly for me)

 

Here is what I did (which is what you suggested): 

1. Went to TT and in the upper right selected "Forms"

2. Found form 8889-T in the list and deleted it (it now has a red ! point next to it)

3. In the upper right selected "Step by Step"

4. Selected "PERSONAL"

5. Selected "Deductions & Credits"

6. Scrolled down and selected "Medical"

7. To the right selected "Visit All"

8. Select YES for Did you have any medical expenses in 2025

9. Check Box Health Savings Account (HSA) - then at the bottom "Continue"

10. The next few questions you have to navigate yourself with your situation

11. When you get to "What type of HDHP coverage did you have during 2025? - select "had  different plan types at different times of the year  (MY OPINION IS THIS IS WHERE THE TT BUG IS - I had been selecting the first option covered by family plan every month)

12. Select continue and the next page in a list of months

13. From the Health plans drop down select your plan for each (all 12 months) month

14. Select continue and get through the next few questions specific to your situation until your get back to the category header "Your 2025 Deductions & Credits"

15. Once you are there go directly to "FILE" and finish filing

Evidently because I have the dreaded "TY25_HSA - There is a potential issue related to HSA, MSA Contributions." error there is no way to e-file my return through Turbo Tax. Why did I buy it then??

Same rejection message: TY25_HSA. Rejected 3 times so far. I'm at a loss how to fix. I've reviewed the 8889 form and find no error. No over contribution 2025 or 2024. Don't know how to fix.

Yeah, I just finished printing the forms and pulling the guide from the IRS (like I've been doing since the 80s) and did not find an issue that would trigger a rejection. I didn't review everything but the I went over the HSA forms pretty thorough.  I'm pretty sure I can go to a competitor and get this done with no issues. Full disclosure, having changed Jobs and putting in two different HSA's I discovered I over contributed by $100 for the year. I answered all of that correctly and decided to ignore the added 6% on that over contribution. But no, I still get rejected and nothing in any TT Knowledge article is helpful at all. Sigh!!! So again, why am I paying the price for this?

Same issue. Stepped thru all HSA forms and no error or issue and keeps getting rejected by fed, state, etc. WTF.

Same issue here.  I reviewed the HSA section multiple times and i do not see any errors.  Refiled three times and rejected three times.

There might be a bug.  TT is checking too many boxes.  Here is what I am trying.  Delete forms 8889.  Go back to the Q&A.  Even if you had the same coverage all year, select No.  Then for each month check off the coverage you had.  At least that's what another user said to do.  Skip the final review as that will just rebreak it.  I resubmitted 1 hr ago and so far, no rejection email/Text.  Yesterday was getting the rejection after 15 mins.  So hopefully this is it.

I followed the same steps posted here and finally got my Federal accepted after 8 attempts of trying everything! Thanks to both this posting and the previous posting by another user.

I had a HDHP for the entire year.

I had to manually delete the form 8889-T form, repopulated it through the interview BUT say instead of marking that i had coverage all 12 months, i had to mark that i had mixed coverage for the 12 months then manually mark the months that i had Self coverage which was all year. I then followed the instructions from this post to go directly to "File" after completing the HSA questions via the interview and resubmit. Federal came back quickly; still waiting on state.

Thanks! After 7 changes to HSA process, deleting, manually adding and getting all rejected, I tried the "had  different plan types at different times of the year " and I think it worked. TT shows Feds accepted, thought I got an IM which says it was rejected. Still pending State and local.

If you are getting the HSA_2025 rejection code when trying to electronically file your return, try using the following steps to clear the internal condition that is causing the rejection code.  These steps have worked for some users to be able to refile with acceptance of their returns.  (See posts above for other user experiences.)


Also, please note that you should not check the box indicating that you are ‘covered by Medicare’ unless you are receiving Medicare benefits rather than having HDHP insurance coverage.  This question is also asking about 2025, not 2026.  So if you are covered by Medicare in 2026, this check box does not apply to you.

 

Use these steps:

 

  1. Go to Deduction and Credits > Medical > 1099-SA, HSA, MSA and click Update
  2. Continue through the HSA section until you reach the page that asks if you were covered by a High Deductible Health Plan in 2025 and answer according to your situation.
  3. Next, check any appropriate boxes on the page that asks Do any of these situations apply to you? (This is where it is important to be accurate about whether you were receiving Medicare benefits during 2025).
  4. The next page asks what type of HDHP coverage did you have in 2025.  Check the box that says ‘I had different plan types at different times of the year’, then click Continue.  
  5. On the next page, select the type of coverage for each month of the year according to your situation.  (If you were covered by Medicare in 2025, indicate that you had Medicare or None for any month you had Medicare coverage.)
  6. Continue through the process to answer the same set of questions for your spouse, if applicable.
  7. Keep clicking Continue until you are back at the main Deductions and Credits page.

 

At this point, you can go through the error check process and refile your return.

Thank you so much for your clear, step-by-step instructions for resolving the TY25_HSA issue. I followed your guidance and my federal tax return was accepted — what a relief!

 

For context, I had called TurboTax support the day before after several failed resubmission attempts. I spoke with two different representatives, neither of whom was particularly helpful. One advised me that I shouldn't have tried resubmitting multiple times, while the other suggested I contact the IRS directly to find out why the return was rejected — and both ultimately recommended I just print and mail my returns.

 

Given that your fix resolved the issue entirely, it's clear to me that this is a bug in TurboTax, not something users should have to troubleshoot on their own. I hope this feedback helps bring more attention to the problem.
