I received a 1099-DIV with an amount of $400 on Line 11 Exempt Interest Dividends. Turbo Tax is asking me for what state the dividends came from. Where can I find that info on the 1099 DIV form?

Even if these dividends or interest won't be taxed on your federal return, we still have to ask about them because most states will tax dividends or interest that are paid by another state. Exempt-interest dividends are usually interest payments on state or municipal bonds.
Your mutual fund broker would provide details on additional states. It's usually listed on the last pages of the statement. If they did not provide a breakdown, you can select "More than one state" at the bottom of the state scroll-down list.
Your home state will tax all the dividends except the dividends from municipal bonds from your state.
