You can use the federal ID number that is listed in box "b" if there is not state ID number listed on your W2 in box "15". Make sure you have your two letter state abbreviation as well.

If you reported an Identity Theft to the IRS in a prior tax year then the IRS would have sent you the 6 digit IP PIN in December or January.
Go to this IRS website for IP PIN information - https://www.irs.gov/identity-theft-fraud-scams/retrieve-your-ip-pin
To enter a 6 digit IP PIN
Or enter ip pin in the Search box located in the upper right of the program screen. Click on Jump to ip pin
Unfortunately we don’t have the option of retrieving our Identity theft PIN number due to “low volumes of staff”. Does this mean we’re pretty much out of luck?
