Both my husband and myself will leave US and therefore we will both be considered as NRA. If we want to buy the medical insruance and pay the medical expense in the USA for each other, I assume there will not be considered as gift. We can pay each other whatever we want. I just want to double check do we need to report it to the IRS? I assume no need, right?

You are correct, there is no need. Funds transferred between spouses is not reported on a tax return.

Correct. No taxes on medical expense paid.
