Do I need to pay state income tax in my old state and the one I currently reside in?

I received a disbursement from an old employers 401k.  My address in Fidelity's system is my parents.  However, I have not lived at that address for 10 years.  I now reside in a different state.  Fidelity said they were unable to update the 1099-R to reflect my current residency.

No, you will only need to include your 401(k) distribution on the state return for the state you were a resident of when the distribution was made (regardless of the state were the contributions was made).

This applies even if the address listed on your 1099-R is your former state of residence.
