Hello - Received $5 for a fractional share cash in lieu, however this was not reported on a 1099B - BUT it was in the misc transactions section on the very last page of a 1099-DIV (just not in any of the critical boxes of page 1 of the 1099) So is this still required to be reported ?
Also if it is required, I was able to find the cost basis from the spinoff company investor website calculator - should I use the actual cost basis (it was another small #) or is it just safer and cleaner to report 0

Yes, even though it is a small amount, you should report it. Because it is cash in lieu of a fractional share, you need to determine your basis in that fractional share and when you know the basis, you will know whether you have a gain or loss from that fractional share. Your sale proceeds are $5.00, you just need to determine the basis.
You could enter this fractional share information manually in the Investments and Savings section of TurboTax online, or in the Investment Income section if using TurboTax CD/download.
