Can't get custom CSV file to load...incorrect headers

I used Nexo to earn interest on my crypto "investments".  I downloaded my Nexo "transactions" in a CSV file.  I downloaded the "Custom CSV template" and carefully/painstakingly migrated my data into the correctly file.

When I attempted to upload the CSV file it said that my headers were incorrect.

Bearing in mind i did not edit the column headings at all.  I thought, maybe it's because i changed the column width.   I tried again.  Failed due to "incorrect headings".   Then I considered that it might be due to the interpretation of the CSV in open office.  

I tried uploading the "Custom CSV file" just to see and got a "Malformed CSV File uploaded. " error. 

I just used notepad to open it and there were definitely "tabs" added into my csv.  I massaged the file and replaced them with commas.  This time I got the same "Malformed CSV File uploaded. " error.   so I guess i've made progress.
