Why is Turbotax telling me I had a lapse in health when the only change was adding a baby to the insurance and do I pay a penalty for that?

Be sure you indicated in the Health Insurance section that you were all covered all year (screenshot).

There is no penalty for adding your baby. The baby is also considered 'covered all year' if they were born anytime in 2018.
