Entered 1099-R for my spouse and totals appear OK, however, during NJ state review, program prompts for Simple IRA, first for myself and then for my spouse for the same rollover amount, as if 2 rollovers were entered.

It depends. To clarify, what types of questions are the prompts asking for? Did you indicate that the rollover was to a Simple IRA when the interview question appeared while you were preparing your state return?
Simple IRA was not checked on the original entry of my spouse's 1099-R form.
The state review prompt in question appears to be confirming this, however, it is prompting twice for the same form, once for myself and once for my spouse. I answered the prompt properly twice in order complete the taxes. The extra prompt seems to be a bug in the software.
All of the final figures appear OK so I am proceeding.
