1099-SA, I dont know what to put for payers name, payers address, or federal ID number

You should receive Form 1099-SA from the trustee that holds your HSA or MSA account.

Payers name and payers address is in the "TRUSTEE'S/PAYER'S name" box at the top left of Form 1099-SA.

The federal ID number is in the "PAYER'S federal identification number" box under the Trustee/Payers name.
