There is no form 1088 SA but there is a 1099-SA which is a form for distributions from an HSA account. I assumed you meant a 1099-SA and the instructions I gave you are for where you enter your HSA distributions.
Do you receive social security? You would only have the SSA-1099 if you receive social security. if this is the form you are asking about the SSA-1099 would be entered under Income and Expense > scroll to Retirement Plans and Social Security > Social Security (SSA-1099, RRB-1099) > Click on Start to enter the SSA-1099.
