Where do I get the 8889-T form?

Do you mean by where to enter info so to let the program create the form?

Where do I get the 8889-T form?

You don't get a form 8889-T, TurboTax creates it from information on your W-2 and from information you enter in the HSA interview (Federal->Deductions & Credits->Medical->HSA MSA Contributions).

Then this form, if it is required, is added to your tax return package and filed with the IRS.
