Should we get a 1099-SA
My wife has an employer funded HSA. It was noted on her W-2. Last year, healthcare providers submitted charges against the account for payment. Were those charges considered distributions? And should she have received a 1099-SA from her employer?

Yes, any time a payment was processed through the Health Savings Account, it would be considered a distribution.
Yes, you should have received a Form 1099-SA that would report the total distributions for the year. It would come from the financial institution that administers the HSA instead of from your spouse’s employer. If there is an online account where you are able to log on to see the balance of the HSA, you may be able to find the Form 1099-SA there and reprint it.
