Should I enter the totals in medical fees or just the part I payed out of pocket?

Per IRS Publication 502, just the part you paid out of pocket.  Costs that are paid by insurance, discounts, etc. are not deductible medical expenses.  

 

Note that in order to claim medical expenses you must itemize deductions and only the expenses over 7.5% of your AGI are deductible.
