It appears that Charity Limit Form (Charitable Deduction Limits Worksheet for Current Years Deduction line 39 is using data from cells other than those it lists. Ln 39 says to add 10,14,21,27 and 36. In my case all of them are zero (0) except 14 which is 272. I would expect 39 to be 272. Instead it is 134,892. As a result my carryover is incorrect.
Also, line 43 says to enter the amount from line 41 on the Schedule A line 11 dotted line to the left. The number in 43 is not the number on the dotted line to the left of Schedule A Line 11.
This is serious as the IRS computer will reduce Charitable deductions if the number on the dotted line is incorrect.

More research - Line 39 includes Carryover Allowed from 2020,. Both our 2021 30% and 100%/60% both have carryover from 2020. The total of the two is equal plus 272 is equal to the amount in line 39. lines 38-41 do correctly give your allowed deduction for the 100% level. The calculation of the carry forward to to 2022 appears to be correct as well.
So the biggest problem other than the words not matching the actual data in the form, is that the number on Schedule A dotted line next to box 11 is incorrect. It should be equal to line 41 on the worksheet. It is not. This is the critical part as far as the IRS computer is concerned.
After trying unsuccessfully to duplicate your situation in a test file, it could be helpful to examine the situation in more detail if you would be willing to share a diagnostic copy of your tax file. The diagnostic file will not contain personally identifiable information, only numbers related to your tax forms.
