Do not report the reimbursement on your NJ tax return. HSA distributions are not taxed by New Jersey. Money that goes into the HSA is after tax in NJ, and is not deductible in NJ, so you already paid NJ tax on the money in your HSA. You don't pay NJ tax on it again when you take it out of the HSA. Just ignore the HSA completely on your New Jersey tax return.
