Form 8889 confusion - Line 1/Line 9

On form 8889 do I fill my contributions that get taken out of my paycheck on line 1 and my employer's match on line 9?

I'm asking about this if I'm filling out the tax form manually not using turbotax.

Form 8889 line 1 is a checkbox, not an amount. Do you mean line 2?

Line 2 is for contributions that you (or someone on your behalf) made, other than contributions through your payroll. All the contributions made through payroll go on line 9, including both the amount deducted from your pay and any additional amount contributed by your employer. Line 9 should be the amount shown in box 12 of your W-2 with code W.
