Previous employer refuses to provide my 1099-SA. What can I do?

A previous employer makes tax forms available via ADP (common payroll processer).  My W2 is on there, and I have asked repeatedly for my 1099-SA.  They insist it is also on the ADP site, and it is not.  I have shown proof, and they refuse to provide help or my 1099-SA.

Your 1099-SA should come from the account holder (usually a bank or plan administrator) and the only way ADP would have the form is if ADP is the plan administrator.  You likely have a login for the bank or plan and should look there for an electronic copy of your 1099-SA.

ADP appears to have a marketing agreement with HealthEquity, so HealthEquity might be your HSA custodian.
